Terms and Conditions of Booking
1. Fees.
Fees quoted are programme fees inclusive of GST. Fees include all assessment material and registration of credits, if applicable. Fees are subject to change without notice. Instructor/assessor expenses will always be charged. Training manual = available on day, riggers card= included. 1 day training and 1 day further assessment, (min 4/max 6 people per course). If 1 day course, costs for assessing on your sites will be included in costs.
2 . Fee Structure. $1100.00 incl GSt per person for 2 day course, $550 incl GST per person for 2-day refresher course

3. Payment.
Delegates will be refused entry to programmes unless payment has been received and banked prior to course commencement. Payment can be made by direct credit to account no: 03-0435-0034111-000. Invoices will be issued on receipt of this booking form.
4. Bookings.
Bookings for programmes will only be accepted by FASET NZ Assoc. when this booking form is returned, with client’s purchase order/no.
5. Pre-programme logistics.
Circumstances may dictate that FASET NZ Assoc. may incur expenses prior to receipt of a signed booking form. This will be confirmed during discussions, however in the event the client alters these verbal arrangements, FASET NZ Assoc. will forward an invoice to cover these commitments.
6. Cancellation by FASET NZ Assoc
We reserve the right to cancel or alter the dates of courses and the venue. Weather critical courses maybe rearranged, but costs incurred by FASET NZ Assoc. will be the responsibility of the client.
7. Notice of cancellation/change of date by client.
Notice given of cancellation: 7 days or less – 50% of total course fees payable.
Notice given of change of date: 7 days or less – 30% of total course fees payable.
If our instructor attends your premises to deliver programmes, but is unable to do so due to lack of facilities or any other failure caused by the client, fees will be payable in full.
8. Health, fitness and aptitude (where appropriate).
It is the client’s responsibility to ensure all candidates are physically fit and sufficiently mentally equipped to work at height. Participants must have a clear understanding of the English language (both written and verbal) to enable completion of assessments.
9. Programmes.
Programmes supplied by FASET NZ Assoc . are normally carried out during normal working hours. In the event the client requests weekend or out of normal hours courses, a supplement to our normal fees will be applied. Instructor travel greater than 2 hours may incur an overnight charge.
10. On-site facilities.
Where FASET NZ Assoc. trainer attends client’s premises, the following shall be made available (verbally agreed prior to programme start).
Classroom Programmes
- Classroom facilities (desks & chairs)
- Suitable heating/ventilation & power
- A projection screen or white wall
- Welfare facilities
- Parking for the instructor
Practical Programmes
- Classroom facilities (desks & chairs)
- Suitable heating /ventilation & power
- A projection screen or white wall
- Welfare facilities
- Refreshments
- Clearance with other on-site operations for uninterrupted work
Other Requirements
- 6m x 3m x 2m high rig (constructed from scaffolding or timber)
- Appropriate access (powered access/ladders, etc)
- “In test” safety nets to EN1263/1, sized to facilitate practical work
- Candidates must be certified in access equipment
- Access equipment must be fit for purpose
- Attachment systems appropriate for practical work
- Candidates must be suitably attired with appropriate PPE clearance with site management for uninterrupted work
